Can You Fly with Cellulitis? Risks, Precautions, and Insurance

Having cellulitis does not automatically mean you cannot fly. If the infection is mild, you are otherwise well and responding to treatment, travel may still be possible. However, you should seek medical advice if the infection is worsening or you are systemically unwell.

What is cellulitis? 

Cellulitis is a bacterial infection that affects the deeper layers of your skin and the underlying tissue. It typically happens when bacteria enter through a break in the skin, such as a cut, scratch, or insect bite, causing the area to suddenly become painful, hot, swollen, and red. While it most commonly develops in the lower legs, it can appear anywhere on the body and requires prompt treatment with antibiotics to prevent the infection from spreading.

How flying affects cellulitis: the primary risks 

Cellulitis most commonly develops in the lower legs. Air travel changes the physical environment around you, which can directly affect how your body deals with an infection:

  • Increased swelling and discomfort: Sitting still for long periods can contribute to swelling in the legs. If cellulitis is already causing swelling and tenderness, a long journey may make the affected limb more uncomfortable
  • Blood clots: Long periods of immobility during travel can increase the risk of deep vein thrombosis (DVT), particularly in people who have other risk factors. Some symptoms of DVT, including pain, warmth and swelling of a leg, can also resemble cellulitis, so new or unexpectedly worsening symptoms should be assessed.

Checklist: 5 Tips for travelling safely with cellulitis 

If you are safe to travel, a few simple adjustments can make your journey much more comfortable:

  • Pack extra medication in your hand luggage: Always keep your antibiotics in your cabin bag, not your hold luggage.
  • Keep the affected area elevated: Whenever you are sitting at the airport or relaxing at your accommodation, use a chair or a pillow to prop up your swollen limb.
  • Stay hydrated: Drink regularly during your journey to avoid dehydration. If you have broken skin or a wound, follow the wound-care advice given by your healthcare professional and keep the area clean.
  • Preventing another episode while you are away:  A simple bug bite can break the skin and introduce new bacteria, making your recovery harder. Pack a high-quality insect repellent containing DEET and wear long sleeves or trousers if you are visiting a bug-prone destination.
  • Keep moving: Regularly flex your ankles and move the joints around the affected area. On a longer journey, get up and walk periodically when it is safe and comfortable to do.

Red Flags:

Cellulitis can escalate into a serious condition if the bacteria spread into your bloodstream. The following symptoms are associated with situations requiring urgent medical attention:

  • A high temperature, chills, or uncontrollable shivering.
  • A rapid heartbeat or fast breathing.
  • The red or swollen area on your skin is spreading rapidly or developing purple patches.
  • Dizziness, confusion, or feeling faint.

If you need hospital assessment or intravenous antibiotics, or your treatment is being changed because the infection is not improving, discuss your travel plans with the team treating you before flying.

Declaring cellulitis for travel insurance 

When you buy travel insurance, you must declare cellulitis during the medical screening process. This applies whether you are currently taking a course of antibiotics or if you suffer from recurring bouts of the infection.

If you do not declare it and require emergency medical care abroad because the infection has spread, your policy could be invalid. Your insurer may reject your claim, leaving you responsible for expensive hospital fees and repatriation costs.
